REQUEST A FIELD TRIP

STEP ONE:

Please read and acknowledge Section 1: Reservation Agreement before beginning the application process.

It is required for Section 1 to be acknowledged, signed and dated before moving on to Section 2.

STEP TWO:

Section 2: Field Trip/Visit Request Form

It is required for Section 2 to be filled out completely for the form to be submitted successfully. Don’t leave fields blank. Please type, “N/A” or “Not Applicable” for questions that do not apply.

STEP THREE:

Receipt of Confirmation Letter
Once an Educator has scheduled your visit, you will receive a confirmation letter and an invoice via email. Receipt of the confirmation letter and invoice verifies that your visit is officially scheduled.

STEP FOUR:

These forms must be filled out by each adult and youth participant and returned no later than 2 weeks prior to scheduled visit. Return forms in-person at Ford Nature Center or scan and return via email or postal mail (details to follow).
