FIELD TRIP/VISIT RESERVATION REQUEST FORM

STEP ONE:

Please read and acknowledge Section 1: Reservation Agreement before beginning the application process.

It is required for Section 1 to be acknowledged, signed and dated before moving on to Section 2.

STEP TWO:

Section 2: Field Trip/Visit Request Form

It is required for Section 2 to be filled out completely for the form to be submitted successfully. Don’t leave fields blank. Please type, “N/A” or “Not Applicable” for questions that do not apply.

STEP THREE:

Receipt of Confirmation Letter
Once an Educator has scheduled your visit, you will receive a confirmation letter and an invoice via email.  Receipt of the confirmation letter and invoice verifies that your visit is officially scheduled.

These forms must be filled out by each adult and youth participant and returned no later than 2 weeks prior to scheduled visit. Return forms in-person at Ford Nature Center or scan and return via email.

Section 1: Reservation Agreement

Thank you for requesting a field trip or group visit to Mill Creek MetroParks. We are excited to work with you in creating a perfect visit to the park. Whether it's your first visit or you're a seasoned visitor, please read the following as many policies have changed regarding field trips and group visits to the park.

  • Field trips/interpretive tours are scheduled in the order in which they are received and how they fit into our schedule with staff availability.
  • All participants (youth and adult) must have a completed waiver to participate in the activity.
  • A FINAL HEAD COUNT must be submitted three weeks prior to your tour. This is very important because the number of participants influences the number of sessions we will need to divide your group into as well as the number of staff required for your scheduled visit. Staff schedules are made up two weeks in advance.
  • Tour fees and signed waivers are due no later than two weeks prior to your visit. It is the organizer’s responsibility to be sure that every person (adults and youth) attending has submitted a program waiver form. If we have not received those items, we will open your date(s) back up for other groups.
  • Registration Fees (minimum fee is $50.00)
    • Regular Youth: $4.00
    • Regular Adult: $5.00
    • Mahoning County Youth Discounted: $2.00
    • Mahoning County Adult Discounted: $3.00
  • For youth groups, one adult (teacher, chaperone, aid, nurse, etc.) is free for every 10 paid youth participants once the $50.00 minimum is met.
  • All tours will include an outside session, so it is important that you are dressed for the weather. Rain, snow, cold or hot temperatures—we will still do the outside activity. Please watch the weather reports leading up to your tour and communicate with your participants and/or their parents to dress for being outside. If there are thunderstorms or other dangerous conditions, we will cancel your visit. Every attempt will be made to reschedule. If that is not possible, your fees will be refunded. Fees are only refunded when we have canceled a tour.
  • Plan on arriving at your selected facility ten minutes before your scheduled start time. We often have multiple groups coming and cannot delay beginning your program. If you are late arriving, your tour will be shortened to still end at the scheduled end time. Please call 330.740.7107 ext. 129 as soon as you realize you are going to be late so that we can begin to alter your schedule. Groups that are 30 minutes late will have the tour canceled without refund.
  • We will make every effort to customize topics/standards covered during your tour. Please communicate special requests at the time that you are booking the trip to allow staff adequate time to prepare the focus.
